The Expanded Care Management Team (ECMT) hub is a significant part of the overall mission and priority strategic aim for the St. Luke's Health System. Care management is about 'knowing' and 'connecting' patients with needed resources, enhancing self-sufficiency, and supporting equitable access to excellent care.
