Overview The Clinical Treatment Coordinator (CTC) will partner with the physician and interdisciplinary team to navigate the patient/family through treatment episode, including assessment, treatment planning, discharge planning and community re-entry. This includes, but is not limited to: managing patient/family expectations, responding to the crisis of hospitalization, supporting the discharge and community re-entry needs and monitoring and managing patient length of stay and avoidable days.
Demonstrates clinical competence in the treatment planning process.
Demonstrates clinical competence to assess individual patient needs.
Demonstrates clinical competency to partner with inter-disciplinary team in the treatment planning process.
Demonstrates competence in developing a clinically relevant transition/discharge plan.
Demonstrates clinical competence in conducting group therapy, family therapy and psycho-education groups.
Demonstrates knowledge of utilization review and collaborates with the Utilization Management Specialists to insure the ongoing, comprehensive monitoring and evaluation of patient care, by concurrent and retrospective review to assure appropriate reimbursement. Demonstrates the ability to advance evidence based practice by utilizing standards, guidelines and pathways for care delivery. Incorporates data and information into practice to continually improve care and practice so that patient outcomes are enhanced. Promotes a culture of evidence based practice by participation in any of, but not limited to, the following activities:
Responds and participates in Behavior Control Situations (BCS) and CPR. Required Education
