The Nurse Navigator will guide behavioral health patients through the health care system, serving as a single point of contact for referring physicians, clients, caregivers and human service organizations to provide resources and assistance with services offered within South Shore Health System and its partnering health care community. You will provide brief interventions in acute care, primary care and other community settings; educate clients on the importance of mental health services; navigate clients to a long-term provider of behavioral health care; and serve as a liaison between clients, healthcare providers and human service agencies to reduce barriers to care.
BSN with a focus in Behavioral Health; MSN preferred
Current Massachusetts RN licensure
Demonstrated proficiency in acute care nursing
3-5 years of experience in behavioral health nursing
Knowledge of local and state regulations and local community resources
RN-BC Psych certification preferred
